Note: We do not offer Dealer Discounts on Service Center Charges

After you submit the form, you should receive a copy of the form at the email address provided. Please check your SPAM box if you don’t see the email within a few minutes. Print two copies of the email, one for your records, and put one in the box with your items that need to be serviced.
Shipping Guidelines:
The product should be properly packaged with special emphasis on the inside of the box, surrounding the entire product.
Products should be double-boxed.
Packages should be insured for your protection.
